2/15/2024 EnchantaVaughn Approved Behavior: Client expressed a positive attitude while engaging in conversation with mom and sibling, upon QMHPâs arrival. Parent communicated that client is allowed to play outside every day after school to accommodate her social needs but under the conditions she finishes her homework and assigned chores. Client presented herself with a pleasant attitude and did not complain or show frustration towards this decision. Client understands that everyone is there to help and support her and that it is okay to ask for help and mother reports behaviors can last up to 10 minutes when she cannot have her way she will lash out on others. Behavior: Client was able to cope and relax in her environment and complete her assignment while QMHP collaborated with her mom regarding her ISP. Client seems to be interested in working on herself, as evidenced by successfully completing the intervention and modeling good self-talk and affirmations. Mother reports when she cannot speak positive about herself she will lash out on others and yell at which behaviors last about 20-30 minutes in duration. Intervention: QMHP talked to client about her confidence levels and how they can affect her thoughts and behaviors. QMHP encouraged client to utilize positive self-talk as a means of decreasing anxiety and managing ADHD. QMHP reminded client of her strengths and abilities. QMHP supported client as she identified areas she has been working on that allows her to feel productive and in a good mood. QMHP reviewed and discussed the importance of displaying respectful behaviors within the home and community. Intervention: QMHP processed with client about psycho educational activity while assisting with directions. QMHP explained the importance of the activity. QMHP provided feedback to client in regard to questions asked. QMHP encouraged client and parent to take notes during the session to instill repetition. QMHP provided feedback and applied active listening skills throughout the session. QMHP discussed with the client triggers to different emotions and strategies and interventions to assist client in managing those emotions. Response: Client displayed self determination to stay focused and pay attention in the session. Client reduces sadness and finds more excitement in learning about her family history and mental health. Client discussed mental illness and its causes as a family. Client displayed interest in learning about ADHD, as evidenced by her asking questions and committing to her treatment. Client understood effective problem-solving skills to help solve or overcome a problem or stressor the she is currently facing. Response: Client has become more involved in age appropriate social activities and was given positive reinforcement for these choices. Client demonstrated expectations and responded well to feedback. Client reported a decreased stress level as he continued to express positive words of affirmations. Client and mom understood the contract for safety, due to implementing appropriate behaviors in the home. Client modeled the positive reinforcement provided by her counselor, motivating her to be good for the rest of the day.
